Minnesota State Accident Form

Every driver in a crash involving $1000 or more in property damage, injury or death must complete the State required citizen's report form. This form may also be picked up at the City of White Bear Lake Police Department.

Once completed, this form must be submitted to the MN Driver & Vehicle Services (DMV):
