The National Aging in Place Council created Act III: Your Plan for Aging in Place, a downloadable template to assist in planning to age in place.

The Lifetime Home

An interactive resource for older adults, families and professions to learn how to make the home a safer and more supportive place for a lifetime.  Created by the USC Leonard Davis School of Gerontology.

Twin Cities Habitat for Humanity

Twin Cities Habitat for Humanity's Age Well at Home™ program offers a resource list and a downloadable Aging In Place Guide.  Additionally through their healthcare partners, patients may be referred to Twin Cities Habitat to Humanity for home modifications which are prescribed to preserve their ability to stay in a healthier, safer home.
