Resource Referral & Service Experts for Aging in Place

Below are a list of agencies and non-profits that are able to make referrals to services that make it possible to age in place.

White Bear Area Senior Program

Part of the White Bear Lake Area Schools District, the White Bear Area Senior Program provides education, recreation, volunteer opportunities, resource navigation and support services for older adults and their families.  Additionally they have created resource guides for transportation, services and housing.

Senior LinkAge Line

A free service of the state of Minnesota that connects older Minnesotans and their families with the help they need.

Consortium of Lake Area Senior Services (CLASS)

Community professionals working to educate, organize and advocate to improve the quality of life for seniors in the community, including connecting older adults and their families to services in the community.

Minneapolis/St. Paul Chapter of Age in Place

Professional individuals who can provide a service or make a referral to assist individuals and families reduce stress when a family needs assistance to help them live where it is safe and comfortable.

MnCHOICES Assessment

MnCHOICES is an in-person visit, usually where you live that integrates assessment and support planning for Minnesotans  of all ages who have any type of disability or need for long-term services and supports.  There is no cost to you for the assessment.  Contact the county where you area for an assessment referral or Senior LinkAge Line (1-800-333-2433).  Click here for Ramsey County information or here for Washington County information.
